Evidence of Immediate Travel:

Provide a copy of one of the following (Applicant's name must appear on the travel documentation):

• A copy of your round trip ticket indicating date of international travel or;
• A computer generated travel itinerary or e-ticket indicating date of international travel.
• In the case of death in family: please show documents to prove it.
• In the case of ill family member(s): please provide a copy of doctor's note.
