The flight crew will be in Brazil from July 1st to July 7th, 2005. The purpose of the visit is to transport corporate executives from our headquarters in Chicago, IL to Brazil and back. While in Brazil the crew will not be providing any technical assistance.

Due to the nature and frequency of our visits to Brazil, we would like to request a 5-year multiple entry visa issued for each of our flight crew members.

AIRLINES, Inc. (Your Company Name) will financially guarantee the expenses of our flight crew and corporate aircraft while in Brazil.
